Age Limit: Candidates age limit should be between 21 – 32 years for Sl No. A & B (01 to 03) as on date of advertisement, maximum 68 years for Sl No. A (04) & between 21 – 35 years as on 01-02-2016 for Sl No. B (04). Relaxation of age will e applicable to ST, SC, SEBC, Women, PH and Sports Person quota candidates as Govt of Odisha rules.
Educational Qualification: candidates should have passed in General Nursing & Midwife/ B.SC. Nursing for Sl No. A (01 & 03), B (01), passed in Laboratory Technician course from any three Medical Colleges of the state or any AICET approved institution for Sl No. A (02) & B (02), MBBS/ Assistant Surgeon for Sl No. A (04), Diploma in Pharmacy for Sl No. B (03) & Graduation with minimum 50% marks in aggregate and have passed PGDCA/ DCA for Sl No. B (04).
How to Apply: Eligible candidates may send their application in prescribed format with self attested copies of mark sheets, qualification certificate, date of birth certificates, 02 passport size colour attested photograph, experience certificate, caste certificates, residential certificates, handicapped certificate & self addressed envelope with stamp of Rs. 22/- in an envelope should be super scribed with “Post Applied for —————-” by registered post/ speed post to the Office of the Chief District Medical Officer, Sambalpur, DHH, Modipada, Sambalpur, Pin-768 002 on or before 25-02-2016.
